The ExxonMobil Medical Plan (EMMP) is made up of two components - a POS II
option and a Health Maintenance Organization (HMO) option. The benefits of
each EMMP option are described in individual Summary Plan Descriptions (SPDs).
The SPDs on this page are summaries of your benefits under the EMMP HMO option

Aetna or CIGNA HMO
Participants
If you participate in an Aetna or CIGNA HMO,
your SPD is comprised of the following:
- Aetna HMO Benefits
Information Booklet or CIGNA
Network Medical Benefits Option Information Booklet, a user’s guide
highlighting plan participation rules that are governed by ExxonMobil
Corporation and descriptions and highlights of HMO provided health care
services and provision including a Benefit Summary.

HMO provider directory, a listing of physicians available in the HMO network,
obtained from the HMO administrator (
Aetna or CIGNA).
The benefits information booklet for your HMO is the same regardless of where
you live. However, your provider directory is different depending on your home
address zip code.
All Other HMO Participants
If you participate in any other HMO (e.g., Kaiser Permanente, Preferred Care
Rochester, etc.), your SPD is comprised of the following:
- ExxonMobil Medical Plan
Fully Insured HMO Option Information Booklet

HMO specific benefits information (e.g., Certificate of Coverage, Member
Handbook, etc.,) obtained from the administrator (i.e. the insurance company)
of your HMO and

HMO provider directory obtained from the HMO administrator (i.e. the insurance
company).
The ExxonMobil Medical Plan Fully Insured HMO Option Information Booklet is
the same regardless of which fully insured HMO you are enroll in. The specific
HMO benefits information and HMO provider directory depend on the option that
you enroll in and is different depending on you home address zip code.

This information does not comprise all the details about the SPD for your HMO
option. You must obtain specific HMO information (e.g., Certificates of
Coverage, Member Handbooks, etc.) and provider directories directly from your
HMO Member Services by calling the number on your ID card. Contact information
is also listed in the Information Sources Section of the Aetna and CIGNA
Benefits Information Booklets and Appendix A of the ExxonMobil Medical Plan
Fully Insured HMO Information Booklet.
